Immigration-Related Forms

Students and Scholars

The forms are delivered in Adobe Acrobat .pdf format, and may be completed in one of two ways:

  • Fillable form – This method enables you to click on a field and type the required information directly into the online form. Once you've entered your information, print the form and submit it to the U-M International Center along with any other required documents. You cannot submit .pdf forms to the U-M International Center online.
  • Printed form – This method enables you to simply print and complete the information by writing in your information neatly in pen. Please print and sign the form, and submit it to the U-M International Center. Your signature is required on all forms.

F-1 Students

FormDescription
I-20 Request Form to Replace Lost, Stolen, or Damaged I-20 This form is used when a student needs a replacement I-20.
I-20 Request Form for Extension This form is used by students who require additional time to complete academic program requirements and are requesting an extended I-20.
Academic Advisor Recommendation Form for I-20 Extension This form is to be filled out by an academic advisor who is recommending that a student be issued an extension I-20.
I-20 Request Form for Use by Dependents This form is used to request new I-20s for dependents who wish to enter the US or change status to F-2 status.
Initial I-20 Request form for Continuing Students This form is used by students who are continuing U-M students but who need an initial I-20. Do not use this form if you are a new U-M student; contact your admissions office for your initial I-20.
Financial Resources Statement This form must accompany certain I-20 request forms. It is used to certify sufficient funding for the issuance of a new I-20.
Financial Affidavit Each individual contributing financially to the named student's education must complete the Affidavit of Financial Resources.
I-20 Travel Signature Drop-Off Form This form is needed to request official I-20 authorization to reenter from a trip outside the U.S.
Enrollment/Currency Request Form The enrollment request form is used to ask for a letter verifying your full time enrollment at UM and/or to receive a recommendation that you be granted foreign currency exchange for the amount of funding necessary to continue your studies at U-M.
Reduced Course Load Form This form is used to request approval for Reduced Course Load (RCL) enrollment from the U-M International Center.
Full Time Equivalency for Graduate Students This form is used to request a certificate of full time equivalency from the U-M International Center.
F-1 Severe Economic Hardship Request Form This form is used to request recommendation for severe economic hardship employment authorization.
F-1 Transfer Out Form This form is used to notify U-M of your intent to transfer your F-1 immigration status to another school. The U-M International Center uses the information to update your SEVIS record with the name of the school to which you will transfer and your transfer release date.
F-1 Transfer Out Checklist This checklist guides you through the process of officially transferring from U-M to another educational institution in the U.S.
F-1 International Student Departure Form This form is used to notify the U-M International Center that you are leaving U-M or the U.S. temporarily or permanently. The U-M International Center uses the information to adjust or end your SEVIS record.
I-20 Request for LSA Undergraduate Cross-Campus Transfer This form is used by Undergraduate LSA students who have received approval from the LSA Advising Center to transfer from another UM school to LSA and need an I-20 to reflect the new academic program. This I-20 must be obtained before beginning study in LSA.
I-20 Request for LSA Undergraduate Reactivation This form is used by LSA students who took an academic break for two or more years and are now returning to U-M LSA. Students using this I-20 request form must have already received confirmation of the reactivation from LSA Academic Advising.
Remove Dependent This form is used by F-1 students who wish to remove their F-2 dependents from their I-20.
Update Financial Information This form is used by F-1 students who have a significant change in their funding situation and wish to be issued an updated I-20 reflecting this change.

J-1 Students

SEVIS DS-2019 Request Form This form is used to request that changes be made to your DS-2019, for example, extension, adding dependents, editing financial information, etc.
SEVIS DS-2019 Travel Signature Drop-Off Form This form is needed to request official DS-2019 authorization to reenter from a trip outside the U.S.
On-Campus Employment Authorization for U-M Sponsored J-1 Students This form is used to request on-campus work authorization.

J-1 Scholars

Hosting a J-1 Scholar to U-M (Authorized Use Only for U-M Departments)

Form Description
SEVIS DS-2019 Request For Use by J-1 Scholars This form is used by professors, research scholars, short-term scholar, or specialists. It is used to request a new DS-2019 if you need to replace a lost form, add dependents, or edit financial information.
SEVIS DS-2019 Travel Signature Drop-Off Form This form is needed to request official DS-2019 authorization to reenter from a trip outside the U.S.

J-1 Visiting Scholar Early Departure Form This form is used to notify the U-M International Center that you are leaving U-M or the U.S. The U-M International Center uses the information to adjust or end your SEVIS record.
J-1 Exchange Visitor Information Form The exchange visitor information form is for newly invited exchange visitors to provide information to their host departments. Do not submit this directly to the International Center; return it to your inviting academic department.

J-2 Dependents

Form Description
SEVIS DS-2019 Request for use by J-1 Scholars This form is used by professors, research scholars, short-term scholar, or specialists. It is used to request that changes be made to your DS-2019, for example, adding dependents, editing financial information, and to apply for DS-2019 for J-2 dependents.
I-765 Employment Application Form This form is used to request an Employment Authorization Document (EAD) from the U.S. Citizenship and Immigration Services (USCIS) office.

Faculty and Staff

Permanent Residency (U-M sponsorship only)

Permanent Residency Process Authorization Form
(Authorized Use Only)
This form is used by U-M department administrators to initiate a permanent residency case with the U-M International Center.

For H-1B specialty employment, the department initiates H-1B cases. Refer to H-1B Visas - Temporary Worker in a Specialty Occupation for more information.
Request for Retained Immigration Counsel Form
(Authorized Use Only)
This form is used by U-M department administrators to request an attorney approved by the U-M Office of the Vice President and General Counsel.

UPS Shipping Information Sheet If you are unable to pick-up documents at the U-M International Center, please fill out online & print this form to provide us with shipping information. We will arrange for shipment of the documents via UPS. The cost of shipping is your responsibility, and we have found that the most secure way to pay is with a UPS account number. If you do not have an account, you can easily create one for yourself. Instructions are on the UPS Shipping Information Sheet form.
