F-1 International Student Departure Form

This form allows the International Center to adjust or end your F-1 SEVIS record because you are leaving the University of Michigan and the US temporarily or permanently. It is also recommended that you review the following document: Departure/Transfer-Out Checklist.

  • If you are leaving U-M temporarily (excluding summer vacation), or you are withdrawing from U-M, we recommend that you meet with an International Student Advisor before you submit this form online.
  • If you are graduating from U-M, and your I-20 has not expired yet, submit this form online; you do not need to see an advisor.
  • If you are on post-completion OPT and leaving the U.S., and you will not return to continue your OPT, submit this form online; you do not need to see an advisor.
  • If you are on the UM International Health Insurance plan, your insurance will end at the end of the month that your SEVIS record ends.

  • Your F-1 status and SEVIS record will be terminated while you are gone. Your I-20 will be invalid.
  • One month before your return contact the International Center to initiate reactivation of your I-20 and SEVIS record.
  • You will not be able to re-enter the U.S. if you do not reactivate the I-20 and SEVIS record.
  • Your F-1 status will be terminated, and your I-20 will be invalid. You must request a new I-20 before returning.
  • You must apply for a new F-1 visa to return even if your current F-1 visa appears valid.
  • Your I-20, SEVIS record, and F-1 status will be terminated.
  • You may remain in the US for 15 days from the date of withdrawl.
  • You have a 60 day grace period after you complete your academic program.
