This form is to be used for continuing U-M students and scholars to request a replacement of the DS-2019 due to loss, theft or damage.
Do not use this form if you are a newly admitted student or newly invited scholar. Please contact your admitting unit or your inviting department for a new DS-2019.

DS-2019 processing time is 5-10 business days.
