|Actual Wage Determination Form||
This form is used in the H-1B process by U-M department administrators to document the "actual wage," which is based on internal wage data and on the salaries of individuals who are considered to be “similarly employed” to the foreign national.
|Department INSZoom Authorization Form||
This form is used by U-M department administrators to request access to the INSZoom on behalf of their departments.
|DS-2019 Department Request Form: Extension and Amendment||
This form is used for J-1 exchange visitors who are already here at U-M and need updates and changes to be made to their DS-2019 form.
|DS-2019 Request Form for New and Transfer J-1 Exchange Visitors||
If your department is requesting a new or transfer J-1 program please fill out this form and submit to the International Center after the data has been entered into M-Pathways.
|Employment Change Notification Form||
To be completed when requesting a change to the employment (e.g. salary, title, hours, location, including a transfer to another UM department) of an employee in H-1B, TN, E-3 or O-1 status.
|H-1B/ O-1 Termination & Transfer Form||
This form is used by U-M department administrators to report that an H-1B or O-1 will be leaving the department and/or ending their U-M employment.
|H-1B/ TN/ E-3 Process Authorization Form||
This form is used by U-M department administrators to initiate an H-1B, TN or E-3 visa petition with the U-M International Center.
|Permanent Residency Process Authorization Form||
This form is used by U-M department administrators to initiate a permanent residency case that will be filed by the U-M International Center, i.e. all EB-2 and EB-3 cases for individuals who have teaching duties.
|Request for Retained Immigration Counsel Form||
This form is used by U-M department administrators to submit a request for a permanent residency case (EB-1 and non-teaching positions), O-1 petition or certain waivers of the 2-year home residency requirement.
