Academic Advisor Recommendation for I-20/DS-2019 Extension

This form is to be filled out by an academic advisor who is recommending that a student be issued an extension I-20 or DS-2019.

Academic Training Request for J-1 Student

This form is used by J-1 students to request Academic Training.

Add J-2 Dependent(s) Request

This form is used to request new DS-2019s for dependents who wish to enter the U.S. or change status to J-2 status.

Affidavit of Financial Resources

Each individual contributing financially to the named student's education must complete the Affidavit of Financial Resources.

Continuing Student Request for New DS-2019

This form is used by students who are continuing U-M students but who need an initial, transfer back, or reinstatement DS-2019. Do not use this form if you are a new U-M student; contact your admissions office for your initial DS-2019.

Departure Form for J-1 Student

This form is used to notify the U-M International Center that you are leaving U-M or the U.S. temporarily or permanently. The U-M International Center uses the information to adjust or end your SEVIS record.

Extension of DS-2019 Request for J-1 Student

This form is used by students who require additional time to complete academic program requirements and are requesting an extended DS-2019.

Financial Resources Statement

This form must accompany certain I-20 or DS-2019 request forms. It is used to certify sufficient funding for the issuance of a new I-20 or DS-2019.

Full-time Equivalency Request for Graduate Student

This form is used to request a certificate of full time equivalency from the U-M International Center.

International Health Insurance Plan Change Form

This form is used to request changes to your existing U-M International Student/Scholar Health Insurance coverage, such as a change to the coverage end date, the addition of a dependent, etc.

Off-Campus Employment Based on Economic Hardship for J-1 Student

This form is used to request recommendation for severe economic hardship employment authorization.

On-Campus Employment Authorization for U-M Sponsored J-1 Student

This form is used to request on-campus work authorization.

Reduced Course Load (RCL) Request for J-1 Student

This form is used to request approval for Reduced Course Load (RCL) enrollment from the U-M International Center.

Remove J-2 Dependent(s) Request

This form is used by J-1 visa holders who wish to remove their J-2 dependents from their DS-2019.

Request to Replace Lost, Stolen or Damaged DS-2019 Form

This form is to be used by *continuing* U-M students and scholars to request a replacement of the most recently issued DS-2019 due to loss, theft, or damage.

Social Security Letter for J-1 Student

This letter is signed by the International Center to verify your J-1 status to assist in your Social Security Number application. Please read Getting a Social Security Number and Card.

Standard Letter Request Form

This request form is used to ask for one of three standard letters: immigration status verification letter verifying your U-M visa status (for students whose I-20/DS-2019 was issued by U-M); estimated expenses letter indicating the amount of funding necessary to continue your studies at U-M; or guest student letter indicating your eligibility to attend another school during your summer vacation.

Transfer-out Request & Checklist for J-1 Student/Scholar

This form is used to notify U-M of your intent to transfer your J-1 immigration status to another school. The U-M International Center uses the information to update your SEVIS record with the name of the school to which you will transfer and your transfer release date. The checklist guides you through the process of officially transferring from U-M to another educational institution in the U.S.

Travel Signature Request for J-1 Student/Scholar

This form is needed to request official DS-2019 authorization to reenter from a trip outside the U.S.

Update Financial Information Request on DS-2019

This form is used by J-1 students who have a significant change in their funding situation and wish to be issued an updated DS-2019 reflecting this change.

Update Personal Information Request on DS-2019

This form is used by J-1 students who have a change or correction in their personal information (name, citizenship, birthdate) and wish to be issued an updated DS-2019 reflecting this change.

These additional clearance checks may take several weeks to several months to process. There is no way to circumvent or expedite this process.
